Fraxel RE:PAIR Success Strategies by Dr. Vartanian
Introduction
The Fraxel re:pair is the latest and best laser from the Fraxel line of lasers for reduction of facial lines, etc., and should not be confused with or compared to the original Fraxel (Fraxel SR 1500) - a laser that delivers much less noticeable improvement than the RE:PAIR. So, Fraxel does not = Fraxel Re:pair. Two different machines, two different price points for the patiet - and in my opinion: a great difference in results! - with the Fraxel Re:pair being a much superior machine in terms of what patients want, RESULTS.
Overview
The Fraxel re:pair laser is the only CO2 laser on the market that delivers the efficacy of traditional bulk ablative lasers—withoutthe prolonged downtime, and minimized risks or complications. Unlike traditional CO2lasers, which perform a thin, wide areas, uniform and general ablation of the entire skin surface, the Fraxel re:pair laser uses microscopic treatment zones (MTZs) to ablate tissue deep within the dermis. Even though the laser does not protect the stratum corneum, the unique Fraxel technology spares much of the surrounding healthy tissue in the dermis. This results in immediate coagulation, tissue shrinkage and skin tightening.
Powerful and versatile—Treat several indications with a single system
Due to the fractional technology, the Fraxel re:pair system can be used for both superficial skin conditions and deeper damage. And since different energy levels can be selected, including treatment levels and number of passes, each treatment can be custom tailored to each patient’s needs. Key attractive features of the Fraxel re:pair system:
- Treats up to 4 times faster than any other ablative fractional device. The exclusive Intelligent Optical Tracking® system adjusts to hand speed and ensures treatment is uniform and fast. A typical full-face treatment takes just 30-40 minutes.
- Delivers the broadest range of fractional ablative treatment parameters. The intuitive, touch-screen interface guides allows for various parameter selections—superficial to deep (5-70 mJ) and mild to aggressive (5-70% coverage). The system optimizes the microscopic spot size for each energy level, maximizing penetration while minimizing bulk heating in surrounding tissue.
- The only fractional ablative device with FDA clearance for vascular dyschromia. In addition, Fraxel re:pair treatment is cleared for ablation and resurfacing of soft tissue, rhytides, pigmented lesions, wrinkles, fine lines, furrows and textural irregularities.
- Ease of use. Fraxel re:pair is a single-handed, single-operator treatment. Plus, it is the only fractional ablative device with built-in smoke evacuation, minimizing plume and smell—and eliminating the need for staff support.
- Safe choice. The unique Fraxel scanning technology, combined with the precision and effectiveness of a CO2 laser source, reduces the degree of risk for lines of demarcation or long-term side effects compared with traditional CO2 lasers.
- Clinically tested technology. The Fraxel re:pairlaser is supported by extensive research with more than three years of clinical follow-up in over 2,500 patients and with 5,000 histologies.
Dr. Vartanian's Treatment Method:
The only resurfacing technology used by Dr. Vartanian is the FRAXEL re:pair due to its safety, efficacy, ease of use, and high patient satisfaction. On the subject of high patient satisfaction, local facial nerve blocks are quite important in allowing a comfortable treatment experience for the patient.
Dr. Vartanian combines low dose oral analgesics, with topical numbing creams, and complete facial nerve blocks to administer and essentially pain-free treatment experience for the patient.
Risks and complication can be minimized:by using more conservative setting vis a vis the patient's skin type, not overlapping beyond the number of passes, use of Medrol dose pack in the post-treatment period, use of Vigilon dressing in the first night after more aggressive treatment settings, and being extra careful in the neck and mandible areas. Dr. Vartanian has only treated Fitzpatrick skin types I, II, and III, and in these patients, no skin color changes have been noted.
In our practice, very high patient satisfaction rates have been documented with the Fraxel re:pair. Patient selection is important, facial skin type analysis and realistic expectation management is a must. As an example: An older patient with deep folds and wrinkles, and tremendous skin laxity who needs at least a facelift, plus multiple other procedures, must be educated about the expected improvements from one Fraxel repair treatment session.
In summary, in properly selected patients, and with an experienced practitioner, the Fraxel re:pair laser can produce great results, with moderate downtime and almost no complications.
